CDH Privacy Policy – Compliance with Law 25 (Québec)

1. Who We Are

The Centre of Dreams and Hopes (CDH), located in Montréal, Québec, provides educational, recreational, therapeutic, and community services to neurodivergent individuals and their families. As part of our commitment to privacy and transparency, we collect and manage personal information in accordance with Law 25 (An Act to modernize legislative provisions as regards the protection of personal information).

2. What Personal Information We Collect

Through the CDH Membership Form, we may collect the following information:

  • Client’s full name
    • Date of birth
      • Gender identity
        • Phone number(s) and email address
          • Parent/guardian contact information
            • Living situation and home address
              • Diagnosis and medical/mental health conditions
                • Requested services and programs
                  • Emergency contact information

                    3. Purpose of Collecting Your Information

                    We collect personal information to:

                    • Register clients for CDH services and programs
                      • Communicate with clients and families about activities, services, and events
                        • Ensure client safety, accessibility, and appropriate support
                          • Process membership payments
                            • Provide third-party referrals (e.g., for speech or occupational therapy)
                              • Support internal data analysis and reporting for grant applications and funding compliance (no identifying information is shared in public reports)

                                4. How Your Data is Stored

                                All responses collected through the CDH membership form are stored securely on Google servers at data centres. These servers may be located outside of Québec, including in the United States, Europe, Asia and other countries.

                                5. Security Measures

                                We take appropriate precautions to protect your data. Access is restricted to authorized CDH staff. Google Forms data is encrypted in transit and at rest. We follow best practices for digital data security.

                                6. Your Rights Under Law 25

                                You have the right to:

                                • Access your personal information
                                  • Request corrections to inaccurate data
                                    • Withdraw your consent
                                      • Request deletion of your information (where applicable)

                                        7. Data Retention

                                        Your personal data (as defined in section 2)  will be retained only as long as you have an active file (i.e. membership and use of services). After that period, it will be securely deleted or anonymized.

                                        8. Consent

                                        By submitting the CDH Membership Form, you consent to the collection, use, and retention of your personal information as outlined in this privacy policy.

                                        Photo & Media Policy

                                        As part of our programs, services, and community engagement efforts, the Centre of Dreams and Hopes (CDH) may document activities through photographs and video recordings. These media assets may be used for internal and external communications to highlight our mission, showcase participant engagement, and support reporting to funders.

                                        1. Purpose of Media Collection

                                        Photographs and videos may be used in the following contexts:

                                        • CDH website and social media platforms (Facebook,Instagram)
                                          • Internal reports and grant submissions
                                            • Promotional materials such as brochures, flyers, and newsletters
                                              • Educational and staff training purposes
                                                • Community outreach and awareness campaigns

                                                  2. Informed Consent

                                                  No photo or video will be taken or used without prior informed consent.

                                                  • Clients who are 14 years of age or older and are capable of providing informed consent may sign the Photo & Media Consent Form themselves.
                                                    • Clients under the age of 14, or individuals who require assistance in decision-making due to disability or legal status, must have the form signed by a parent, legal guardian, or tutor.

                                                      Consent is voluntary and may be withdrawn at any time upon written request. In such cases, CDH will cease any future use of the individual’s image and remove it from public platforms whenever possible.

                                                      3. Confidentiality and Use of Personal Information

                                                      CDH is committed to protecting the dignity, privacy, and identity of all participants.

                                                      • We do not publish full names or identifying personal information unless explicitly authorized in writing.
                                                        • Media is used in a manner that respects the values of inclusion, dignity, and safety.

                                                          4. Storage and Access

                                                          All photos and videos are stored securely and accessed only by authorized CDH personnel or designated partners. Files are protected in accordance with Québec’s Law 25 and applicable privacy legislation.
